Abstract
Reply from the Authors: We appreciate the interest of Dr. Friedman in our paper.’ While he agrees with our suggestion that the co-occurrence of secondary mania and hemiballismus after a right thalamic lesion may be explained by a dopaminergic overactivity, he seems to conclude that women are more predisposed than men to develop mania after right hemisphere lesion because they have an asymmetric biochemical distribution of amines in the CNS with increased dopaminergic turnover in the right hemisphere. To examine whether mania after right hemisphere injury is a gender-specific affective disorder, we reviewed the gender of 34 published patients with secondary mania and lesions restricted to the right hemisphere. We found that 28 (82%) patients were men, whereas only six (18%) were women. Therefore, these findings do not support gender disparities-at least in the sense suggested by Dr. Friedman-regarding mania secondary to focal right hemisphere lesions.
