Hydrometrocolpos is dilatation of the uterus and vagina proximal to a congenital obstruction of the distal vagina, secondary to an intact hymen, a vaginal membrane, or vaginal atresia. The lesion appears ultrasonically as a double cystic mass filled with mucoid material and desquamated epithelium.• Although this entity is extremely rare, especially in association with a duplicated uterus and vagina, one should include this anomaly in the differential diagnosis of a painful pelvic mass in an adolescent girl or young woman with ipsilateral renal agenesis. We describe the ultrasonographic findings in a recent case and stress the value of this modality in preoperative evaluation.
