Some studies suggest that statins may be associated with weight loss in Type 2 diabetes patients, while other studies indicate that statin therapy may reduce the favorable effects of bariatric surgery on weight loss and lead to increased fat and calorie consumption.

Statins are widely prescribed medications primarily used to lower cholesterol levels and reduce the risk of cardiovascular diseases. However, their impact on weight loss, especially in patients with metabolic disorders, has been a subject of interest and research. This article synthesizes findings from multiple studies to determine the best statin for weight loss.
Research has shown that statin therapy can influence metabolic outcomes in patients undergoing bariatric surgery. A study involving 1575 patients who underwent various bariatric procedures found that diabetes resolved more frequently in patients who were on preoperative statin therapy compared to those who were not (23.7% vs. 13.7%) . However, patients who ceased statin therapy post-surgery experienced the highest rates of diabetes remission (35.7%). This suggests that while statins may support diabetes remission post-surgery, discontinuing them might lead to even better outcomes.
There is evidence suggesting that statin use might lead to weight gain due to behavioral changes. A study analyzing NHANES data from 1999 to 2010 found that statin users significantly increased their fat intake and calorie consumption over time, leading to higher BMI compared to non-users. This indicates a potential moral hazard where patients might rely on statins as a safety net, neglecting healthy dietary practices.
In a study comparing atorvastatin and rosuvastatin, both drugs were found to lower cholesterol levels and improve glucose tolerance in mice with diet-induced obesity. Additionally, they decreased fat tissue weight and adipocyte size, contributing to an overall tendency for body weight loss. These findings suggest that both atorvastatin and rosuvastatin can be effective in managing weight and improving metabolic health in patients with obesity-related metabolic disorders.
While statins are primarily used for managing cholesterol levels, their impact on weight loss is multifaceted. Statin therapy, particularly when combined with bariatric surgery, can support diabetes remission and weight management. However, the potential for weight gain due to behavioral changes should not be overlooked. Among the statins, atorvastatin and rosuvastatin show promise in improving metabolic parameters and supporting weight loss. Further research, particularly randomized controlled trials, is needed to fully understand the best statin for weight loss and its long-term implications.
