Efficacy and Tolerability of Prinivil (Lisinopril) in Hypertensive Patients

Efficacy in Blood Pressure Control
Comparison with Procardia XL
A study comparing Prinivil and Procardia XL in 135 patients with mild to moderate essential hypertension found that both medications were effective in controlling supine diastolic blood pressure (SDBP). At the end of the titration period, 91% of patients on Prinivil and 95% on Procardia XL achieved SDBP control. By the end of the maintenance period, 79% of Prinivil patients and 80% of Procardia XL patients maintained SDBP control. The mean decreases in SDBP were comparable between the two groups, with Prinivil showing a reduction of 9.6 mmHg at the end of titration and 10.8 mmHg at the end of maintenance.
Monotherapy Effects
In another study involving 20 patients with mild to medium severe essential hypertension, treatment with Prinivil (20-40 mg daily) significantly reduced systolic and diastolic blood pressures. After six weeks, systolic pressure decreased from 169 mmHg to 149 mmHg, and diastolic pressure from 108 mmHg to 91 mmHg. For patients requiring an increased dose of 40 mg, systolic pressure further declined to 140 mmHg and diastolic pressure to 85 mmHg over the subsequent six weeks.
Tolerability and Side Effects
Adverse Experiences
Both studies reported that Prinivil was generally well tolerated. However, the comparison study noted that the number of adverse experiences requiring discontinuation was significantly higher in patients receiving Procardia XL compared to those on Prinivil. The monotherapy study did not observe any marked side effects in patients treated with Prinivil.
Metabolic Effects
Glucose and Lipid Metabolism
The monotherapy study also investigated the effects of Prinivil on glucose and lipid metabolism. It was found that Prinivil had a positive effect on glucose metabolism, particularly in patients with impaired glucose tolerance. Mean blood sugar levels during the second hour of an oral glucose tolerance test (oGTT) decreased from 9.81 mmol/l to 7.28 mmol/l after 12 weeks of treatment. Additionally, immunoreactive insulin levels dropped from 82.49 microM/ml to 34.94 microM/ml. However, there were no significant changes in lipid spectrum parameters after 12 weeks of treatment.
Conclusion
Prinivil (lisinopril) is an effective and well-tolerated medication for the treatment of mild to moderate essential hypertension. It demonstrates comparable efficacy to Procardia XL in blood pressure control, with fewer adverse experiences leading to discontinuation. Additionally, Prinivil positively impacts glucose metabolism without significantly altering lipid profiles. These findings support the use of Prinivil as a reliable option for managing hypertension, particularly in patients with concurrent glucose metabolism concerns.
