Prediabetes, a condition characterized by elevated blood glucose levels that are not yet high enough to be classified as type 2 diabetes (T2D), poses significant health risks, including the progression to T2D and the development of cardiovascular complications. However, emerging research suggests that prediabetes is a reversible condition, particularly through intensive lifestyle interventions (ILI) and, in some cases, pharmacological treatments.
Several studies have demonstrated the effectiveness of ILI in reversing prediabetes. The Pathobiology and Reversibility of Prediabetes in a Biracial Cohort (PROP-ABC) study highlighted that participants who engaged in a structured ILI program, which included calorie restriction, physical activity, and meal replacements, showed significant improvements in glucose regulation. Approximately 43.5% of participants reverted to normal glucose regulation (NGR) within 12 months of starting the intervention, regardless of the duration of prediabetes before the intervention .
Similarly, the Beijing Prediabetes Reversion Program (BPRP) aimed to evaluate the efficacy of lifestyle modifications and pioglitazone in reverting prediabetes to normoglycemia. The study found that intensive lifestyle changes significantly contributed to the reversion of prediabetes, underscoring the importance of early and sustained lifestyle interventions.
While lifestyle changes are the cornerstone of prediabetes management, pharmacological interventions can also play a role. The BPRP study included pioglitazone as part of its intervention strategy, which, when combined with lifestyle modifications, showed promise in reverting prediabetes to normal glucose levels.
