Abstract
Aim: Case presentation of an adult patient with severe periodontal and esthetic endangerment, treated with skeletal anchorage systems and minimally invasive partial fixed appliance in order to obtain an improvement of interdental papillae tropism and an increase of hard tissue around an implant site. Materials and methods: In V.R., 50 years old patient, affected by severe periodontal disease with bone loss around the extruded element 1.2 and mild anterior crowding with loss of interdental papilla, a partial appliance, with two miniscrew implants as anchorage placed in the retrocanine region, was used to align the anterior teeth and extrude 1.2 until the expulsion. Incisors anatomy was modified in order to improve the tropism of papillae. Molars and premolars were intentionally excluded from the appliance. Results: About 18 months of orthodontic treatment allow us to improve the pre-implant conditions increasing the bone level in 1.2 site. The anterior alignment created an adequate and symmetric space for the implant therapy, also enhancing the patient's aesthetics Conclusions: Whenever it is possible, it can be strategic to use partial fixed appliances even associated with temporary skeletal anchorage systems that are more comfortable for the patient, make easier to maintain a better level of hygiene and reduce the risk of undesired alterations of posterior occlusion.
