Abstract
ZusammenfassungMelagatran ist ein direkter Thrombininhibitor. Es handelt sich zusammen mit seiner peroral wirksamen Vorstufe Ximelagatran um neu entwickelte synthetische Dipeptide mit hohem antithrombotischen Potenzial. Sie weisen eine lineare Dosis-Wirkungs-Beziehung auf, haben eine kurze Halbwertzeit und zeichnen sich durch eine vorteilhafte therapeutische Breite gegenüber herkömmlichen Antikoagulanzien (Heparine, Vitamin-K-Antagonisten) aus. Die Ergebnisse von klinischen Studien zur Prophylaxe und Therapie von Thromboembolien sind viel versprechend. Die Anwendung von Melagatran und Ximelagatran wird im perioperativen Management an Bedeutung gewinnen und damit für den Bereich der Anästhesiologie und Intensivmedizin von besonderem Interesse sein.AbstractMelagatran is a direct inhibitor of thrombin and—like its oral prodrug ximelagatran—a newly developed dipetide with high antithrombotic efficacy. They present a linear dose-response, a short plasma half-life and the therapeutic range may be advantageous compared with classic anticoagulants such as heparins or vitamin K antagonists. The results of clinical studies for prevention and treatment of thromboembolic complications are encouraging. The use of melagatran and ximelagatran will gain significance in the perioperative management, thus being of particular importance for anaesthesiology and critical care medicine in the near future.
